As a result, Greg Plunkett in our biochemistry laboratory has conducted a series of experiments over the past 2 years to look at the stability of extracts that are diluted as they would be in patient vials. We have some very sensitive methods that allow us to look at major allergen stability down to dilution 6. As Jorge noted, the sterile diluents used to prepare patient vials all contain 0.4% phenol as is required for all multidose vials intended for injection. The slides I attached show the levels of phenol that allergens are exposed to as extracts are diluted in both 1:10 and 1:5 dilution series. Greg's data have been presented at several AAOA, ACAAI, and AAAAI meetings. He finds that the major allergens are stable in the presence of 0.4% phenol. In looking at diluents, we see absolutely no protective effect of 10% glycerin (despite what King, Mabry, and Mabry say). However, HSA diluent does offer a marked protective effect, likely by keeping the allergens from adhering to the glass vial. Please let us know if we can provide any additional information.
